Mental Health Skill Building

First Home Care offers mental health support services which provide structured activities and ongoing support and guidance toward meeting an individualized service plan with independent living goals. Individual sessions are offered to address deficits in skills which are preventing the individual from independently functioning in the community. Family education to help strengthen communication and interactions is provided when appropriate. Services are recommended for individuals 18 and older, who meet the following criteria:

  1. a diagnosis of Major Depressive Disorder, Bi-polar Disorder, or Schizophrenia
  2. Has been prescribed psychotropic medications in the last 12 months of the assessment date
  3. Has a history of psychiatric hospitalization or crisis stabilization

Mental Health Skill Building offers:

  • Training and reinforcement of functional skills and appropriate behavior to maintain community stability and independence
  • Activities of daily living (ADL)
  • Use of community resources
  • Support individuals on healthy lifestyle choices, and understanding a mental health diagnosis
  • Monitoring of health, nutrition and physical condition
  • Enhancement of social and interpersonal skills
  • Community responsibility
  • Improvement of independent living skills
  • Opportunities for appropriate peer relations and interactions
